چکیده

Left Atrial Appendage Occlusion (LAAO) has emerged as a safe alternative for patients with nonvalvular atrial fibrillation at risk bleeding. Transesophageal echocardiography (TEE) been the gold-standard in guiding implantation and pre- post-operative assessment of device location, compression, stability. The intracardiac echocardiogram (ICE) catheter can also be used to provide imaging guidance during transseptal access deployment LAAO, thus providing an TEE general anesthesia (GA). To review requirements, procedural efficiency, hospital length stay who underwent ICE guided (IG) vs TEE-guided (TG) LAAO. This is retrospective observational study including LAAO between Nov 2020 2022 single center, 4 operators. Variables analyzed included guidance, success, stay, clinical outcomes. Chi squared multivariate t-test were evaluate differences groups. Of total 120 that this period, 64 performed IG 56 TG. Baseline characteristics similar groups (Table 1). Among those had IG, 47(73%) monitored (MAC) 17(27%) GA compared 2(4%) MAC 54(96%) TG group (P<0.0001). Total time EP lab fluoroscopy Acute success was similar, 98% group, 96% (P= 0.9471). Same day discharge successful 22(34%) 17(30%) 0.639). There no statistically significant difference peri-device leak among two follow up regardless type modality (Figure Our results revealed outcomes TE Therefore, highlights periprocedural long-term equivalent Utilizing significantly reduces anesthetic requirements while maintaining safety, success. Consequently, implementation could increase volume capacity electrophysiology labs are currently limited due anesthesiologist availability.Tabled 1Table 1VariableIce-GuidedTEE-GuidedP-valueAge76.475.90.7539CHADS2Vasc4.44.20.5551LVEF58530.0763Pre-procedure CT scan6242<0.0001Monitored Anesthesia472<0.0001General Anesthesia1754<0.0001Acute success63520.9471Total (minutes)1301280.86Total (minutes)1413.80.8796 Open table new tab

A Semi-Automated Algorithm for Segmentation of the Left Atrial Appendage Landing Zone: Application in Left Atrial Appendage Occlusion Procedures

Background: Mechanical occlusion of the Left atrial appendage (LAA) using a purpose-built device has emerged as an effective prophylactic treatment in patients with atrial fibrillation at risk of stroke and a contraindication for anticoagulation. A crucial step in procedural planning is the choice of the device size. This is currently based on the manual analysis of the “Device Landing Zone” fr...

Left Atrial Appendage Occlusion

Left atrial appendage (LAA) occlusion is a treatment strategy to prevent blood clot formation in atrial appendage. Although, LAA occlusion usually was done by catheter-based techniques, especially percutaneous trans-luminal mitral commissurotomy (PTMC), it can be done during closed and open mitral valve commissurotomy (CMVC, OMVC) and mitral valve replacement (MVR) too. Nowadays, PTMC is perfor...

Cost-Effectiveness of Left Atrial Appendage Occlusion

The idea that mechanical intervention to close the left atrial appendage (LAA) might reduce complications from atrial fibrillation (AF) is at least 70 years old. Early attempts to test this idea in humans with surgical ligation were abandoned after only a few years for 2 reasons. First, proponents soon realized that they had no way to choose the candidates believed most likely to benefit withou...

Left atrial appendage occlusion debate revisited.

756 T he left atrial appendage (LAA) has been deemed by some to be our " most lethal attachment, " and the concept of excluding it from systemic circulation has been thought to hold great promise. This concept has been around since 1948, when John Madden 1 proclaimed it a plausible therapy for recurrent emboli in atrial fibrillation. Despite this, higher-quality evidence in the field is only ju...
